By submitting an ACH payment through this form, you ("Customer") authorize AAA Shepherd's Care Home LLC ("Company") to initiate an electronic debit from the bank account you provide for the amount specified.
You further authorize the Company to save your bank account information on file and to initiate future debits for services rendered, including recurring monthly charges, additional fees, or adjustments as applicable.
Each future charge will be for services provided by the Company and you will be notified of the amount prior to or at the time of each debit.
This authorization remains in effect until you revoke it by contacting the Company in writing or by email. Revocation will take effect within 3 business days of receipt and will not affect transactions already in process.
You certify that you are the account holder or an authorized signer on the bank account provided and that you have the authority to authorize these transactions.
If a payment fails due to insufficient funds, a closed account, or invalid account information, you will be notified and no charge will be applied. The Company reserves the right to reattempt the transaction or request an alternative payment method.
Your bank may charge you a fee for a failed or returned ACH debit. The Company is not responsible for any fees imposed by your financial institution.
Under NACHA (National Automated Clearing House Association) rules, you have the right to dispute an unauthorized ACH debit with your bank. You may contact your financial institution to initiate a return within 60 calendar days of the transaction date.
AAA Shepherd's Care Home LLC is not liable for any damages resulting from delays in ACH processing, bank errors, or service interruptions by third-party payment processors. Our total liability for any claim related to a payment shall not exceed the amount of the transaction in question.
We reserve the right to update these terms at any time. Changes take effect upon posting to this page. Continued use of the payment form after changes constitutes acceptance of the updated terms.
